What Is CHAS?

The Community Health Assist Scheme (CHAS) provides subsidies for medical and dental care to Singapore Citizens, including Pioneer Generation (PG) and Merdeka Generation (MG) cardholders, at participating General Practitioner (GP) and dental clinics.

As part of the Healthier SG strategy, CHAS promotes preventive care by encouraging regular health screenings and supporting the management of chronic conditions. Additionally, CHAS works with the Chronic Disease Management Programme (CDMP), allowing the use of MediSave alongside CHAS subsidies for outpatient treatments.

All Singaporeans are eligible for a CHAS card, with benefits tiered according to household monthly income per person or the Annual Value (AV) of the home:

Household monthly income per person = Total gross monthly household income / total number of household members. This can be computed using the calculator provided on the CHAS website.

Annual Value of home = Estimated gross annual rent of a property (excluding furnishings and maintenance fees). It is determined by IRAS and can be checked via myTax Portal.

Health Conditions & Services Covered by CHAS

CHAS subsidies can be used for a wide range of healthcare services at GP clinics, including:

Category Services & Conditions Important Information
Common Illnesses Cold, flu, diarrhoea, skin infection and rashes, urinary tract infection, minor injuries and others. CHAS subsidies are capped at 24 visits per year, across all CHAS clinics.
Chronic Conditions (Simple and Complex) Diabetes, hypertension, high cholesterol, chronic obstructive pulmonary disease (COPD), chronic kidney disease, chronic hepatitis B, gout, osteoporosis, stroke, and more. Simple refers to 1 condition while complex refers to 1 condition with complications or multiple conditions.
Recommended Health Screenings under Screen for Life (SFL) Cardiovascular risk conditions such as diabetes, hypertension, high cholesterol, obesity, as well as cervical and colorectal cancer. CHAS subsidies allow cardholders to pay a fixed fee for screening test(s) and the first post-screening consultation.
Nationally- Recommended Vaccinations Influenza (flu), pneumococcal (PCV13/PPSV23), Tdap (Tetanus, reduced diphtheria & acellular pertussis), HPV, hepatitis B, measles, mumps and rubella (MMR), varicella (VAR). CHAS subsidies are available for citizens who meet vaccination criteria under the National Childhood Immunisations Schedule and National Adult Immunisation Schedule.

How to Apply for a CHAS Card?

You can apply for CHAS online or via a hard copy form:

  • Online: Singaporean Citizens 21 years and above can apply online via the CHAS portal for themselves and their household members. You will need names, NRIC/Birth Certificate/FIN numbers, and contact details of all household members.
  • Hard Copy: Download and complete the CHAS application form. Mail it to P.O. Box 680, Bukit Merah Central Post Office, Singapore 911536, along with any required documents, such as foreign identification documents (e.g., long-term visit pass or work permit) for non-Singaporean household members.

Processing typically takes about 15 working days. If you do not receive a response within this time, check your status online or call the CHAS hotline at 1800-275-2427 (1800-ASK-CHAS).

CHAS Card Validity and Renewal

CHAS cards are valid for two years unless otherwise indicated. To avoid disruption in benefits, renew your card online via the CHAS portal or by submitting a hard copy form before it expires. If you lose your CHAS card, you can request a replacement online or by calling the CHAS hotline. A new card will be issued and mailed to you.
