What Is the SPF Aged Driver Medical Check-Up?

The Singapore Police Force (SPF) Aged Driver Medical Check-Up is a mandatory medical examination required by the SPF Traffic Police (TP) for drivers aged 65 and above.

It ensures that you are physically and mentally fit to continue driving safely, based on key health indicators that reflect overall fitness to drive.

At ATA Medical, we provide smooth, efficient check-ups that comply with SPF's latest guidelines. Our doctors will complete the official form during your visit and return it to you for submission.

Who Needs to Go for an Aged Driver Medical Check-Up?

You are required to go for this medical check-up if:

  • You are turning 65 – A medical check-up is required within 2 months before your 65th birthday to renew your Class 3 or 3A licence.
  • You are already aged 65 or older – A follow-up check-up is required every 3 years (e.g. at 68, 71, 74) to continue driving.
  • You received a Traffic Police notification letter – A check-up is required if the TP requests a medical review for health or licensing reasons.

This requirement applies only to private drivers holding Class 3 or 3A licences and does not apply to vocational licences (VL) such as Private Hire Car Driver's Vocational Licence (PDVL) or a Taxi Driver's Vocational Licence (TDVL).

What Does the Aged Driver Medical Check-Up Include?

The check-up assesses key areas of health that affect your ability to drive safely. It includes:

  • Consultation and Physical Check – Includes a review of medical history, mobility, blood pressure, and coordination.
  • Vision Test – Checks clarity of vision and colour perception. Minimum requirement is 6/12 in each eye, with or without glasses.

A chest X-ray is not necessary unless you are also a vocational driver. Upon completion, you will receive a medical examination report on fitness to drive, which must be submitted to the TP for licence renewal.

What Has Changed for Vocational Drivers Aged 65 and Above?

From 2 February 2026, the Land Transport Authority (LTA) and TP introduced the Harmonised Medical Examination Report (HMER) to simplify the medical check-up process for vocational drivers aged 65 and above who hold a PDVL or TDVL.

What this means:

  • Only one notification letter will be sent
Even if both LTA and TP require a medical check-up in the same year, you will receive just one letter. You will only need to go for one medical check-up to fulfil both requirements.
  • Your clinic will handle the HMER submission to TP and LTA
There is no need to upload the form yourself. An SMS will be sent to confirm once the submission is completed.

These changes apply only to drivers holding vocational licences. If you are renewing a regular Class 3 or 3A licence, you must still submit your medical form to the TP on your own.

How Do You Prepare for Your Check-Up?

To prepare for your aged driver medical check-up, bring the right documents and follow a few simple steps on the day of your visit.

What to bring:

  • NRIC or passport
  • Medical examination form (downloaded from SPF website or received by post)
  • Notification letter from Traffic Police, if you received one
  • Prescription glasses or contact lenses, if used
  • Any specialist reports or memos related to medical conditions
  • A list of your current medications

On the day of your check-up:

  • Wear loose and comfortable clothing
  • Avoid wearing metal accessories in case a chest X-ray is required
  • Prepare a list of your current medications
  • You do not need to fast

How Do I Submit the Completed Form?

Once the doctor completes your medical form, you must submit it to the TP yourself. Clinics are not allowed to submit the form on your behalf for regular aged driver renewals, unless the check-up is done for a vocational licence.

You can submit your form online via SPF E-Services portal using Singpass: https://www.police.gov.sg/e-services

To avoid delays in your licence renewal, ensure that submission is completed at least 4 weeks before your 65th birthday. Keep a copy of the form for your personal records.
